I just received my road incident form from the council to proceed with my pothole claim. In addition to them wanting a copy of my insurance policy the council want proof that I've informed my insurer and proof supppied of their interest.
I'm claiming for a new run flat tyre (£160 ouch!) and a new glove box lid and lock as both where damaged from the pothole.
I don't want to lose over 5 years no claim bonus, can any please advise?

They probably use the same form for all incidents. For incidents that result in damage to the highway (usually a street light) the local authority is entitled to recover their costs from you or your insurers.
If the insurance details are not relevant, then say so on the form. However .... you should check with your insurance company first as some policies require you to inform them of all losses, whether insured or not.
