Alloy Wheel Damaged and tyre split

I was forced by another road user to take evasive action and hit a pothole in the road. The road is a B road and the pothole is 34" long and 22" wide and 4" deep.
This has bent 2 of my alloy wheels and put a cut in one of the tyres which my MOT tester has described as dangerous.

Also i have hit another pothole on the other side and bent the 2 wheels on the drivers side of the car so i now have all 4 bent wheels. I have taken photo's of one of the potholes and also have a video of 2 of my wheels on a wheel balancing machine to show how bent they are.

I would like to know the best way of reporting this and claiming for the damage to all 4 of my alloy wheels and a tyre.
